 SERVICE COORDINATION SERVICES:

After the SC Department of Disabilities and Special Needs (SCDDSN) has determined a person eligible to receive services, a Service Coordinator will be assigned to the case.  The Service Coordinator will complete the SCDDSN Service Coordination Assessment to thoroughly assess a person’s needs.  The Service Coordinator will meet with the person and the family to complete the SCDDSN Support Plan.  The Support Plan identifies the person’s needs and provides a plan for meeting the needs.

The Support Plan is monitored quarterly by the Service Coordinator for satisfaction, usefulness, and effectiveness of services.

The Service Coordinator will be required to meet with the person and/or family at least twice per year.

Service Coordination is an optional service and if there are no on-going needs for monitoring the person and/or family can choose Level II Service Coordination.  The person/family will only be contacted annually.

Services are based on NEED and please note there may be a WAITING LIST for programs or services such as Day Services and Residential Services.
