Who do we serve at    Burton Center?

We provide Service Coordination services to persons residing in Greenwood, Abbeville, McCormick, Edgefield, Lexington and Saluda that have one of the following disabilities:

Ø      Mental Retardation

Ø Related Disability

Ø Autism

 We provide Service Coordination services to persons residing in Greenwood, Abbeville, McCormick, Edgefield, Saluda, and Laurens that have one of the following disabilities: 

Ø      Spinal Cord Injury

Ø Brain Injury

 How does a person become eligible for services under the MR/RD Division?

 REFERRAL INFORMATION FOR MR/RD SERVICES:

For pre-screening and referral, contact the Intake Coordinator at         (864) 942-8925 during the business hours Monday through Friday, 8:00 a.m. – 4:30 p.m.  Once screened in for intake, the Intake Coordinator must complete a home visit as required by SCDDSN during the intake process. Please note that eligibility for services is determined by SCDDSN and this process can take 3-6 months to complete. 

How does a person become eligible for services under the HASCI (Head and Spinal Cord Injury) Division?

 In order to be eligible for services under the HASCI Division the person must have severe and substantial functional limitations as a result of the injury.   

The Burton Center also provides HASCI services to persons in Laurens County.

REFERRAL INFORMATION FOR HASCI SERVICES:

HASCI referrals are pre-screened for the state of South Carolina at the HASCI Intake and Referral in Columbia, SC.   

Please contact 1-866-867-3864 for referrals and pre-screening.  You will be required to leave a voice mail message and someone will call you back to complete a telephone interview.   

If the HASCI Intake and Referral in Columbia, SC screens the person in for eligibility, the screening forms will be faxed to the Intake Coordinator at the Burton Center.  The Intake Coordinator will begin the intake process. 

SERVICE COORDINATION SERVICES:

After SCDDSN has determined a person is eligible to receive services, a Service Coordinator will be assigned to the case. 

The Service Coordinator will complete the SCDDSN Service Coordination Assessment to thoroughly assess a person’s needs.  The Service Coordinator will meet with the person and the family to complete the SCDDSN Support Plan.  The Support Plan identifies the person’s needs and the services and supports to implement to ensure needs are met.

At the annual support plan meeting, the Service Coordinator will provide the person with an estimate of the cost of services they receive, information on what is abuse, neglect, exploitation, and how to report abuse.  The person will also be provided with a copy of the plan.   

The Support Plan is monitored at least quarterly by the Service Coordinator to assure services are received, the person is satisfied with the services, and services are effectively meeting the needs 

The Service Coordinator must ensure each person is afforded a choice of services/ providers, ensure that all human and civil rights are maintained (including services provided by the school system), and consult with other professionals and agencies to ensure needs are addressed.

The Service Coordinator will be required to meet with the person and/or family at least once per year but as often as needed to monitor services.  Monthly monitoring may also be required for persons receiving Home and Community Based Waiver services.   

Service Coordination is an optional service and if there are no on-going needs that require monitoring the person and/or family can choose Level II Service Coordination.  The person/family will only be contacted annually.  If the person receives services through one of the Home and Community Based Waiver programs they are ineligible for Level II Service Coordination.

For those persons receiving Level II Service Coordination, the person will be informed that abuse, neglect, and exploitation information is available upon request, and that the estimated cost of services is also available upon request.

 Services are based on NEED and please note there may be a WAITING LIST for programs or services such as day services, residential services, and/or the Home and Community Based Waiver programs.    

Service Coordination services are accessible 24 hours per day, seven days per week.  Please note in the event of a crisis/emergency situation after the normal business hours of 8:00 a.m. - 4:30 p.m. Monday through Friday, you may contact the Service Coordination Supervisor at    (864)554-2388 or the Special Services Director at (864)554-2398.
