Case Management

The entry point for every person with disabilities is in getting services started and organized. Burton Center provides these individuals and their families with a Case Manager to help obtain the services needed. The Case Manager will get to know the individual and their family in order to help achieve their goals in life.

Getting Coordination Services Started

You will be deemed eligible for services through the South Carolina Department of Disabilities and Special Needs (SCDDSN). Once eligibility is approved, a case manager will help your family complete a SCDDSN case management assessment and a SCDDSN Support Plan. This will help define your needs and implementation methods to ensure those needs are being met.

In order to get your pre-screening and referral, contact our Case Management Department during business hours at (864)942-8900. This process requires eligibility determined by SCDDSN and may take 3-6 months to complete.

Adult Case Management Services

Case Managers work with our clients to meet their needs and work towards their life goals. Our Case Managers monitor services, whether they are in residential sites or occur in-home. Case Managers also ensure that the SCDDSN Support Plan is being followed and that all safety precautions are being correctly followed within the programs.

Child and Young Adult Case Management Services

We offer services to help children and young adults grow to reach their full potential. Some of our clients live at home with their families and receive in-home care, while others live in residential settings. Children who reach the age of 18 are transferred to our adult case management program.

Family-Specific Services

Not only are services coordinated for the individual with intellectual disabilities, but also the family as well. The Case Manager provides services that help the entire family, such as parent or family training, funded in-home support, behavioral consultations, counseling, respite care, and more. Any services that might be needed by the family will be determined when the SCDDSN Support Plan is created. These plans are then analyzed and adjusted on an annual basis.
